Insurance

It can be difficult to understand insurance plans and benefits. Life Balance Acupuncture takes the extra effort to communicate with insurance companies on your behalf, verify your coverage and benefits and develop treatment and payment plans that offer you the best possible care for your needs. This clinic bills insurances out-of-network. Although many insurance carriers cover acupuncture, some such as Medicare do not. Kaiser Permanente does not cover out-of-network acupuncturists. In these cases I offer alternative payment plans that are financially sensible and affordable.

MVA’s (Motor Vehicle Accident)

If you have been in a motor vehicle accident (MVA) in Oregon either as a pedestrian, driver, passenger or bicyclist, you are legally entitled to receive medical care. In the state of Oregon, Personal Injury Protection (PIP) insurance pays medical bills resulting from an MVA. State law mandates at least $15,000 in PIP coverage for you and each of your vehicle passengers for a period of one year after a collision - no matter who was at fault. The law also allows you to seek care from the licensed health care practitioners of your choice. Life Balance Acupuncture will bill the involved auto insurance directly when you provide your claim number, date of injury and contact info of adjuster.

Worker’s Compensation

Life Balance Acupuncture files claims for Worker’s Comp but you will need a referral from your primary care provider. Here is a link you find more information: http://wcd.oregon.gov/worker/Pages/file-a-claim.aspx
